This allows us to verify your insurance benefits before you come in.
Things to bring to your first visit:
On your first visit you will need to bring your insurance card, doctors prescription, and patient forms, which can be filled out online. We ask that you wear loose fitting, comfortable clothes that are easy to move in. For your initial visit plan on arriving 15 minutes early in order to fill out appropriate paperwork, if you haven’t already done so. Before starting, our receptionist will verify your information, explain your insurance benefits and collect any co-pay you may have.
During the evaluation, your therapist will review all pertinent medical history and ask you about your symptoms, more importantly, we will listen to you to ascertain what your needs are. The therapist will take a series of tests and measurements to better understand your limitations and then explain in detail your condition and how therapy will help to restore pain free function. The therapist will then formulate a plan which will be explained to you.
What do we expect from you, and what should you expect from your therapist?
It is important to understand physical therapy will require a commitment from you, a commitment of your time and in some cases, a financial commitment. As therapist we understand this and promise to provide you effective and efficient care delivered with compassion and honesty. We will not be satisfied until you have reached your maximum outcome. We will not waste your time, or make you wait in a lobby. Usually you treatment will start as soon as you sign in and your session will be completed in approximately 45-60 minutes. Most patients require 10-12 visits to progress to pain free function, and your therapist will be able to give you a more accurate treatment duration after the initial evaluation.
How are treatments are determined:
Based on your symptoms and problems that are determined by your therapist, your specific treatments will be broken down into phases which allow us and you to easily monitor your progress. Additionally, you may be asked to perform a home exercise program at some point in your therapy program, which allows you to progress through all the phases as quickly as possible.
