I will get asked often from patients that are contemplating having a having a rotator cuff procedure…. “Greg, Dr. A does his rotator cuff repairs as an open procedure, and Dr. B does his rotator cuff repairs arthroscopically… who should I go with?
I used to say, pick the surgeon that you are most comfortable with… and whatever he does… just go with it. Now after watching two weeks of Wimbledon, I think my answer is changing. Arthroscopic all the way…
Maria Sharapova had a severe rotator cuff tear of her (R) shoulder back in 2007, and had a arthroscopic rotator cuff repair in August 2008. She has battled back to get to the top of the sport again, and although she came up short today in her straight sets loss to Petra Kvitova, she has lots to be proud of.
Rotator cuff tears are the most common tendon injury around the shoulder girdle and often follow an acute trauma or repetitive sports activity… like Sharapova. Not to mention, repetitive activity with awkward mechanics, which I think was the case with Sharapova’s serve… it looked painful when she followed through with her serve…and in all honestly, it’s not that much better now.
They can get back, but in many cases they are never the same type of tennis player.
When I was listening to the post match comments, Sharapova stated that she was the first female player to return from a rotator cuff repair…I’m pretty sure that’s accurate.
Extended rest and strengthening, rather than surgery, is an option for certain rotator cuff injuries. Sharapova tried this initially with very little success, so she went ahead with the arthroscopic procedure.
When you fix a rotator cuff, the healing process tends to lead to more stiffness, the shoulder muscle] has 10 to 15 percent more [stiffness] after surgery. For a typical person, 10 to 15 degrees is not a problem, but for a pitcher or a tennis player, it’s night and day.”
With a complete tear of a rotator cuff, the odds were usually 50-50, of getting them back on the court or the mound. The success rate is not one that instills a lot of confidence, and that’s why athletes are wary when it comes to taking a chance on attempting to return from rotator cuff surgery.
So even though she lost the final today, I’m very happy for her making it back to the top of the sport, and hopefully it instills more confidence in high level overhead athletes, particularly tennis players and pitchers, that a return to high level athletics is very possible after rotator cuff repairs.
